discharge from skilled nursing, short-term rehabilitation or the hospital.
You will get the same trusted care you’d receive in one of our care communities—right in the comfort
of your own home. Whether you live in a private home or apartment, or in one of our independent living, personal care or supportive housing communities, we bring our skilled professionals directly to you.

Skilled Services We Bring to You
We provide you with the services your doctor orders, including:
Skilled Nursing
Registered nurses (RNs) and licensed practical nurses (LPNs) who visit you to ensure you understand the medications your doctor prescribed upon discharge, and will work with you to meet your goal and make sure you know how to manage your health conditions at home. Nurses also can provide:
• Wound care
• IV and infusion therapy
• Ongoing communication with your physician
• Chronic condition education and management
Physical Therapy (PT)
Physical therapists can help you build strength and balance to aid your recovery progress. They also can provide:
• Fall risk assessments and education
• Transfer and gait training
• Strength training
• Home exercise programs
Occupational Therapy (OT)
Occupational therapists can help make you more independent in your activities of daily living (ADLs). They also can provide:
• Home safety assessments and education
• Modifications and tools to make tasks easier
• Education and training on ADLs
Speech/Language Therapy
Speech/language therapists can treat speech or vocal issues, but also can help you with:
• Feeding and swallowing issues
• Cognitive issues
• Memory impairment
Medical Social Work
Licensed social workers can connect you with helpful community support and services offered by other agencies, including:
• Meals on Wheels
• PACE (Pennsylvania’s prescription drug program)
• Office on Aging
• Support groups
• Transportation
Home Health Aide
Home health aides provide intermittent assistance with personal care and activities of daily living.
What You Can Expect
When you select Presbyterian SeniorCare Home Health as your provider, you’ll receive:
• An assigned case manager, who will assemble the team of caregivers that will visit you at home, based on your doctor’s orders. Our goal is to keep you healthy and comfortable at home, minimizing the need for ER visits and hospitalizations.
• A first-visit assessment by each discipline your doctor has ordered. Each caregiver will submit a report on your condition to your insurance to ensure that the services you need are covered.
• An individualized care plan will outline your needs, goals and expectations, as well as the services you’ll be receiving and how often you’ll be receiving them.
• Consistency among your care team, so that we can get to know you personally and develop a relationship with you. That way, we can recognize changes in your health or mental status and act quickly to prevent a decline.
